Description

To develop a working relationship with clients prior to and post release. This may involve visiting the client in prison, meeting them at the gate on release or establishing contact following their release. To support clients in the community and to work towards their ‘action plan’ in liaison with the Community Chaplain. To encourage clients to take ownership of their ‘action plan’.To arrange post release meeting with relevant agencies and support networks (e.g. housing,employment) and accompany clients to appointments where necessary. To keep clear and comprehensive records of the work undertaken and share these with the
Community Chaplain. To complete ‘follow up’ and admin. tasks. To commit a miinimum of 2 hours per week to the role.
